About the position

The Sacramento Fire Department is launching a new program called Mobile Integrated Health (MIH), and we're excited to share that the job posting is now live through PlanetBids. MIH is an innovative, community-based initiative and a key component of our alternative response framework. The MIH program is a part-time or contract-based opportunity, ideal for advanced providers looking for flexible, meaningful work that directly addresses the opioid crisis, frequent ED utilization, and systemic barriers to care.

 

We are seeking collaborative clinical partners who are interested in helping to evolve and modernize our service delivery model beyond traditional clinical settings. The program pairs Nurse Practitioners (NP) and Physician Assistants (PA) with a Firefighter Paramedic to deliver high-impact, patient-centered care directly in the community.

 

In this role, you will deliver high-impact, patient-centered care managing low-acuity and pain-related complaints, monitoring patients enrolled in managed care plans, providing Medication-Assisted Treatment (MAT), and engaging high-utilizer populations to reduce avoidable emergency department (ED) and EMS utilization. You will work alongside fire-based paramedics and community partners to close critical gaps in care and improve outcomes for vulnerable populations.

 

Because this solicitation is being conducted through a Request for Proposal (RFP) process, we will host two virtual pre-proposal meetings on January 5 and January 7 to review the program and application details with potential applicants.
