Subpoena Service: Sacramento Fire Department Medical Records

Subpoenas for medical records related to a response by the Sacramento Fire Department should be directed to ChartSwap. Requester's will need to register, the following information is required:

Visit ChartSwap

  • Completed and patient signed form (online form through ChartSwap)
  • HIPAA Authorization to Release Medical Records Valid I.D. (California Driver's License, California I.D., Military I.D. or U.S. Passport)
  • If the patient's signature is not available, legal documentation such as a subpoena, power of attorney, or letter of representation is required and must accompany the request.

Medical document pick-up is no longer available at the Fire Department. 

Subpoenas for 911 call records should be directed to the Sacramento Regional Fire/EMS Communications Center. Contract the center directly by phone 916 228-3070 for specific service instructions.

Subpoena Service: Citywide

Subpoenas for records, other than those of the Sacramento Police Department, and medical records of the Sacramento Fire Department are accepted by the Office of the City Clerk as outlined below. This includes subpoenas for records, employee records, worker’s compensation, and similar subpoenas for records.

  • By Mail: Subpoenas may be mailed to the Office of the City Clerk 915 I Street, Sacramento, CA 95814
  • In Person: Subpoenas may be deposited in City Clerk's drop-box located on the 1st floor of City Hall at 915 I Street, Sacramento, CA 95814. The City of Sacramento only accepts subpoenas delivered in person via the drop box in lieu of requirement for in person service with signature.
  • By email: The City of Sacramento does not accept subpoenas via email, except by court order or with approval of counsel.

All fees are due in accordance to applicable California and Federal Codes, and should be securely attached to the subpoena filing. Checks and money orders should be made payable to the City of Sacramento. 

NOTE: Employees seeking of copies of their own employee records should contact the City of Sacramento Human Resources department via email at: employment@cityofsacramento.org. Authorized agents seeking records on behalf of City employees must include a signed verification letter with the request for records.
