How do I obtain a Card Room employee permit?

To obtain a card room employee permit you will need to complete an application (SPD 384) and for new employees only, the Applicant's Descriptive Information (SPD 950), and pay the card room permit fee at:

Sacramento City Hall
915 I Street, Room 1201
Sacramento, CA 95814

Once the permit fee has been paid, set up a fingerprinting appointment with the Police Department at 916-808-0780. Take the paid receipt, completed forms, 2 passport-size color photos to the fingerprinting appointment located at:

Sacramento Police Department
5770 Freeport Blvd, Suite 100
Sacramento, CA 95822

Approved permits will be issued by the Sacramento Police Department and mailed to the card room.

How do I renew an existing permit?

Applicant will need to take the completed application (SPD 384), along with 2 passport-size color photos to the Sacramento City Hall address listed above and pay the permit fee. City Hall will forward renewal documents to the Sacramento Police Department, who will approve and issue the renewal permit, which will be mailed to the card room.

What if an employee transfers from one Card Room to another ?

To change card rooms you will need to go to the Sacramento City Hall with the completed application (SPD 384), 2 passport-size color photos and pay the transfer fee.
