Medical incident records requests

If you are seeking medical records regarding response from the Sacramento Fire Department on behalf of yourself or a direct family member, contact Ambulance Billing at (800) 906-6552.

If you are a third party seeking medical records on behalf of someone else, Sacramento Fire Department medical records may be requested through ChartSwap. There is a $30 processing fee due at the time of request. Medical document pick-up is no longer available at the Fire Department. If you have any questions about medical records you may contact the Sacramento Fire Department at any (916) 808-1347.

To obtain medical records you will need to register. What you will need to register:

• Completed and patient signed form (online form through ChartSwap)
• HIPAA Authorization to Release Medical Records
• Valid I.D. (California Driver's License, California I.D., Military I.D. or U.S. Passport)
• If the patient's signature is not available, legal documentation such as a subpoena, power of attorney, or letter of representation is required and must accompany the request.
